Hi all,
This my need:
The agents must read a survey to the called party and set an answer. Is there any way to save these answer in a form in vicidial?

You can try use custom fields in your list and capture the inputs in custom fields.

I think I haven't so much free fields

Go to System Setting, enable Custom List Fields, go to Lists, click on List Custom Fields, click on the MODIFY FIELDS link of a list that you want to add fields to, add as many fields as you like.

I've done what you said but when i go in the campaign of the list i've add custom fields I don't see anything

UPDATE
When i enter in the campaign, if I go on "form" page i see nothing (a light orange page) and if I return on the "script" page I see also the light orange page without any information.

Otherwise, if I'm in call I see my custom field on form page but when i return on the script page I see the orange page without any info.
Could you help me please?
