Two simultaneous calls on an agent meetme

I have a blended campaign, which makes automatic calls and receives calls from a DID.

However in some situations the agents receive two simultaneous calls in the meetme, we don't know how reproduce this.

These agents are logged into the campaign and associated with a inbound group.

Any idea what can cause this and how to solve it?

I would recommend upgrading to a more recent svn/trunk revision of stock VICIdial. GoAutoDial hasn't been upgraded in years and there are many bugs that have been fixed in that time.
