If you have set the caller id name in the dial plan before sending the call out to the carrier, this will likely be the cause of this.
Vicidial uses the caller id name field to track calls, if you need to set caller id name before sending the call out, see the loopback-no-log context in extensions.conf or test adding a p-asserted-identity header to see if your carrier overrides the caller id when it's present.
Other things to check are your dial prefix matches that set in your carrier entry.
