It must be interconnectivity error. This cause indicates that an interworking call has ended.

Means the system was incapable of creating that channel, which could be for a host of reasons. Configuration errors (does that channel Type even exist? are enough channels of that type available or have you exceeded a limit?) or networking or overload.
