8366 survey not working when dialing a lead

Hello, and once again, i am requesting for help , if any one can , regarding an issue i have with survey script.
i had explaint before all the problems happening with survey 8366 when entered into VAD.
however, no successfull explanation had been found yet to this issue, with i think it could be a minor problem.
can any one kindly tell me what is wrong ?a software bugg or simple a missed configuration in my side ?
if you need to check any log regarding this issue, please verify what i had enter into the previous topic i had made, and please repply me back in case you have a solution. Any help will be kindly apreciated.
