While building a new production server on CentOS 4.3 and Asterisk 1.2.11 and VICIDIAL 1.1.12, I encountered this bug:
http://bugs.digium.com/view.php?id=7803
Once I compiled and installed Asterisk 1.2.10, the problem went away.
So....don't bother with 1.2.11 yet!

We have been using 1.2.11 in production for two days. I did have a weird crash at Asterisk-launch yesterday on one machine, but I could not duplicate it and the other server with 1.2.11 on it runs just fine.
astGUIclient/VICIDIAL doesn't really care what Asterisk version you use. Heck, it'll still run fine on CSV_2003_October code.

Ah, I do have a more unique setup. Try making the phones use diff codecs.
My setup is a bit different than most people. I have integrated Asterisk with Zultys MX250. The MX250 only understand ulaw, so I'm going from one codec to another, which causes problems. Just giving everyone heads up on this bug.
