I have an intermittent problem with being unable to load new leads. I'm using the standard VICIDIAL format, and CHECK FOR DUPLICATES BY PHONE IN LIST ID. It loads very quickly, and says that all the records are duplicates. No list exists, or has ever existed, with this list id. When I create a list with that id, it says there are 0 leads. It doesn't happen all the time, but I have been able to reproduce it several times over the last few days.

We have made several fixes in 2.0.4.1rc5 in the list loader, I would suggest upgrading to that and see if the problem still exists.

Is there a maximum number of records that can be in a list? I've loaded the new version, and it is working pretty well, but if the list is too large we just get an error that it doesn't have the correct number of fields.

There is a limit on the size of the lead file. This is determined by the php upload limit on your system. You can raise if you like in the php.ini file on your system. Generally though if you have a lead file larger than 16 megs you are better off using the command line list loader.
