Recording file name oddity

I'm having a weird problem for one rep who uses Vicidial. The campaign that our reps use are set up so that the recording file names are done in the FULLDATE_CUSTPHONE_AGENT format.

For this one rep, sometimes when he starts a recording, Vicidial uses the phone number from the lead he was previously recording, not the one he's currently in-call on.

From his user stats:

You can see that #35 uses the phone number from the previous call of #36 in it's file name, even though he is on a new lead with a different phone number. Ditto for #37 and #38. This problem isn't happening with any of the other reps. What might be causing this?

If he is clicking on START RECORDING before he has actually received the call, then this can happen.
