load new leads problem

i was unable to load the leads properly ,after loading .csv file i am getting the following result

if it is wrong please tell how to upload a sample lead (step by step)

Hi,
the error says duplicate phone - what does your phone_number column look like? Make sure that there are no duplicate entries.

Check your CSV file. What separator does it use? Make sure it's a comma (",") seperated file.
