most of the time this error stems from using the wrong login/password in the wrong place.
be careful that you are using the PHONE login and PHONE password (and reverify it from the Admin->Phones page where it says "login/password" on the entry you are using) on the FIRST screen and then the USER login and USER password (also reverify these from the User menu entry).
99 times out of 100, that's the problem unless you have a bad install.
Which brings up the next point:
when you post, please post your entire configuration including (but not limited to) your installation method, vicidial version and build, asterisk version, telephony hardware (model number is helpful here), cluster information if you have one, and whether any other software is installed in the box.
