New beta 3 release of 2.0.3

Major changes from 2.0.3b2 release:

- Add a "dialed_number" and "dialed_label" variables to the web form output of
the number that was last dialed for that lead and the number field (MAIN, ALT,
ADDR3).

- Add memo and last_called_on fields to the vicidial_campaigns and
vicidial_lists tables for notes and basic information.

- Add a timestamp field in vicidial_campaigns that would log when changes have
been made to the table. Then add an option to update the vicidial_campaign_stats
if this field has a date in the last 90 seconds.

- Add a field in vicidial_campaigns that would force AST_VDadapt to refresh the
vicidial_campaign_stats.

- Have the web-based lead loaders default the phone_code to "1" if none is
specified for a record.

- Added ability to change the height of the conference span in vicidial.php
depending on whether the Alt Dial and/or preview checkboxes are visible.

- Add ability to override campaign recording for a single agent in vicidial.php.
This means adding a new field to vicidial_users and functionality to
vicidial.php and admin.php.

- Add option for logging to queue_log database table for QueueMetrics reporting
system compatibility. The settings for QueueMetrics basic DB connections have
been added to a new "System-wide Settings" section in the admin.php page, the
system_settings table.

- Add more dial statuses for vicidial_campaigns, or break dial statuses off into
a separate TEXT field as the auto_alt_dial_statuses are .

I have deleted the 2.0.3b3 zip file and replaced it with a 2.0.3b4 zip file because there was a bug in the installer. All should work well now.
