Please post your Vicidial Version with Build (and thanks for posting your Installer version of GoAutoDial! very helpful). This information appears on most admin pages (although this particular distro may have "omitted" the word Vicidial which is not cool, LOL).
Campaign->Detail View->Routing Exten (click on the ( ? ) and read the options, one of which is "Survey", choose that Routing Exten).

Do Not make any changes to the survey. Use the default settings. Test it. If it works, make ONE change at a time and test it again. This will "baby step" you to a functional Survey of your choice without paying anyone (like us) $99 to set it up for you. LOL
Read all the options on the page (and press the ? buttons a lot and read them). It's not simple but if you are in the call center business it should not be all that hard, either. Especially if you follow the instructions above and run the Default (example) survey campaign before attempting to make any changes.
If you screw it up and can't get it working again ... make a new campaign and it will have the default settings in it again!
Serioulsy consider upgrading to the latest version of Vicidial (even the 2.6 developer trunk, which is quite stable at the moment) and consider Vicibox 4.0 (another installer, just like GoAutoDial but supported by The Vicidial Group and many others, a wider technical support group than GoAutoDial ... but otherwise pretty much the same thing, just an installer).
